Epic AI, Cerner AI, and monitoring systems generate alerts constantly. This volume trains you to dismiss signals that matter. Your physical assessment of a patient catches what algorithms miss, but that skill atrophies when documentation and risk flagging become automated. Cognitive sovereignty means staying the source of clinical truth, not just the person who validates what the machine says.

Tool names in this checklist are examples. If you use different software, the same principle applies. Check what is relevant to your workflow, mark what is not applicable, and ignore the rest.

Protect Your Assessment Skills from Atrophy

Resist Alert Fatigue and Dismissal of Real Risk

Keep a simple tally for three shifts. Write down the alert type and what you did. This reveals whether you are dismissing whole categories of warnings. If you silence all Cerner sepsis flags, you need to change the threshold or your assessment process.

Go to the alert settings or speak to IT about why it triggered. False alerts are usually fixable. Silencing them trains your brain to ignore the signal type permanently.

Decide which AI alerts you will always investigate, which ones you will check only if your own assessment agrees something is wrong, and which ones your unit should turn off or reconfigure. Write this down. Share it with colleagues. Revise it when your practice changes.

If the AI system generates a note saying the patient ambulated well, but you did not see them walk, delete that sentence. Your signature on the record means you assessed it. AI cannot do your assessment for you.
Flag contextual information in your documentation that metrics cannot captureintermediate
Write down why a patient is refusing early mobilisation, which family member is actually driving decisions, or what the patient said about their experience. This information shapes how the next nurse will interact with them.
Review at least one AI-generated note you did not write before handing over to another nurseintermediate
If a covering nurse or AI system documented something about your patient while you were present, read it. Correct it if it is wrong. This stops errors from propagating through the medical record.
Document disagreement with an AI assessment in your own wordsintermediate
