When these systems fail in conditions outside their training data, no engineer on the shop floor has the judgement to diagnose what went wrong.
These are observations, not criticism. Recognising the pattern is the first step.
Maintenance teams follow AI recommendations because they come from expensive enterprise systems. When a critical machine fails between scheduled intervals, workers cannot explain why the model missed the risk because they never learned to read the signal inputs.
The fix
Require your maintenance planners to document the three sensor readings or historical patterns that triggered each recommendation before work orders are issued.
Technicians with years of hands-on experience with bearing wear and shaft misalignment are reassigned when AI takes over condition monitoring. When the system encounters a novel failure mode, no one remains who can hear what the machine is telling them.

AI systems optimise for cost and lead time by concentrating orders with lowest-cost providers. When that supplier has a fire or quality failure, the organisation has no alternative because no procurement officer maintains knowledge of second-tier suppliers.

Require your procurement team to place at least 5 percent of orders with secondary suppliers every month, regardless of cost difference, and document why those relationships matter for resilience.
Systems like Siemens AI and SAP AI can optimise production settings faster than humans can, but when quality drops or equipment breaks unexpectedly, there is no record of what parameters changed or why. The next shift cannot diagnose the problem.
The fix
Configure your AI systems to log every parameter adjustment with the optimisation objective it was targeting and the measured improvement, then review these logs with your process engineers weekly.
Palantir and SAP AI demand forecasts can be wrong when suppliers themselves are signalling tightness or new capacity. Manufacturing teams discover they cannot source materials only after the AI-driven purchase orders fail.
The fix
Before committing to quarterly procurement levels based on your AI forecast, phone three of your top five suppliers and ask whether they see the same demand picture, and document any disagreements.
AI can suggest route changes, supplier switches, or inventory level adjustments in seconds, but it cannot account for negotiated contracts, quality relationships built over years, or the cost of switching. Engineers implement changes without understanding the trade-offs.
The fix
Require any supply chain recommendation that involves changing suppliers or routes to include a cost-benefit summary that shows current versus proposed total cost of ownership, including switching costs.
